REMINDERS
- Each time you renew a license, be sure to use the
most recent licensure forms. You can download them from www.de.state.oh.us
and search for licensure applications.
- The renewal/transition form must be completed and signed. Be sure that your superintendent does not sign the section that asks for the Employing Ohio School District Superintendent Signature. This section must be signed in the TCESC office.
- Write your personal check or money order payable for the correct amount to
Treasurer, State of Ohio. Your name must appear on the check.
- Write your proper mailing address on your renewal application.
- All teachers must complete a new IPDP each time they receive a new license. The IPDP should be done immediately after receiving the license.
- Do not staple personal checks to applications - use a paperclip instead.
- The only time that you are required to send transcripts to the TCESC Human Resources office
are for one-year temporary renewals and supplemental renewals.
- Anyone requesting a refund for any reason should attention their letter to: Debra Sarver/Accounting Department, Department of Certification and Licensure, 25 S. Front Street, Columbus, OH 43215
